Explicit Consent Statement

Patient Satisfaction Survey

Within the scope of the patient satisfaction survey provided by Özel Dentgrp Ağız ve Diş Sağlığı Polikliniği Sanayi ve Ticaret Limited Şirketi (the “Clinic” / Dentkm), I hereby give my explicit consent to the processing and storage, by the Clinic as data controller, of:

• the answers I submit in the survey form,
• any full name, phone number, e-mail address and country details I voluntarily provide, and
• limited technical records for transaction security (for example, a hashed form of my IP address)

for the purposes of measuring patient satisfaction, improving service quality, handling requests / suggestions / complaints, and contacting me for follow-up where necessary, in accordance with Law No. 6698 on the Protection of Personal Data (the “Law”) and the transfer principles set out in the Clinic’s Privacy Notice.

I confirm that I have read the Privacy Notice; that I am aware of my rights under Article 11 of the Law; and that I may withdraw this consent at any time without giving a reason. Withdrawal requests may be sent to the KEP address dentgrp@hs01.kep.tr or to the Clinic’s postal address for written applications.

Completing the survey is voluntary. Providing contact details is optional. Even if I do not give communication consent, my survey answers may still be used for satisfaction measurement on an anonymous / de-identified basis.
